Request for Proposal

If you would like Dunbar, Bender & Zapf, Inc. to provide a proposal on administration for your plan, please download both of the following files:

	Employee Census
	Retirement Plan Questionnaire

The first file represents a blank census listing in a Microsoft Excel format. Please complete the list to reflect all employees who currently perform services for your business. For each employee, please list his/her name, date of birth, date of hire, date of termination (if applicable during current plan/tax year), social security number, estimated wages/compensation, 401(k) salary deferrals (if applicable), the number of hours completed (if less than 1,000) for this period, and the owner/officer breakdown of the company.

The second file is a blank questionnaire. Please complete all of the items on the form for our records. You may wish to consult your tax advisor on some of the questions, including Item No. 3 regarding Controlled or Affiliated Service Groups.
